If you are an organization interested in joining the Collaborative, please carefully read the Charter here.
Once you have read through the Charter, please complete the Charter Agreement Form linked here and below which will enter you for consideration as a member of the 12-to-24 Collaborative. You will hear back shortly about when your application for membership is reviewed and approved.
